  • Context and justification
  • On 20 June five cases of polio (VDPV1) were confirmed following testing of samples taken during the national polio vaccination campaign in April. This follows a case in October 2014 and another case at the end of January 2015. The confirmation of the earlier cases suggested a risk of a prolonged circulation of the virus in Madagascar, and warranted an immediate response to the outbreak. A social profile of the affected family and community members provided important insights for development of an intensified C4D emergency response plan to create an enabling environment for repeated polio campaigns.

    With confirmation of the new cases the virus has now been found in the north, south, east and west of Madagascar and detailed laboratory investigations of the second case indicated that the virus had been circulating in the country without detection for over 3 years.

    The unprecedented scale of this emergency has been a major challenge for Madagascar which has very limited health infrastructure and services and limited experience and capacity to respond to polio outbreaks. Its emergency response so far has included one round of SNIDs in December 2014 and another round of NIDs in April 2015. In May inactivated polio vaccine (IPV - injectable) was introduced during the biannual mother and child health week. The Global Polio Eradication Initiative has now advised that there should be further rounds in addition to the 9 rounds organized from 2014 to 2016:

  • March 2017: national campaign
  • October 2017: Sub-national campaign
  • As part of the emergency response plan the communication and social mobilization strategy was developed for the past rounds of vaccinations has been adapted for each subsequent round to meet the evolving needs. It aims to increase community understanding of polio and the importance of immunization, to explain the need for repeated campaigns and use of both OPV and IPV and to increase uptake and demand for polio vaccines during routine immunization. So far, efforts have been two-fold: 1) to support interruption of the outbreak; and 2) to support outbreak prevention and preparedness. This is being done through:

  • Sustaining achieved gains to date (build on existing communication strategies, C4D assessments and tools such as training materials, key messages etc).
  • Continuing to address gaps (reaching missed and absent children; hard-to-reach)
  • Building a platform for long-term and integrated success (strengthening local partnerships; use of locally appropriate communication channels and strategies; increased visibility and EPI/polio identity)
  • Due to the increasing number of polio cases and the subsequent need for a large scale, rapid and effective response the C4D section is experiencing greatly increased demands for support for repeated rounds of vaccination campaigns. The consultant will support the C4D section in strengthening the sub-national C4D response plans.

  • Objectives
  • Overall objectives
  • Provide technical support to ensure the successful implementation of the communication and social mobilisation aspects for repeated polio vaccination campaigns in Madagascar
  • To ensure that all additional communication and social mobilisation activities in the regions where cases were found (such as agreements with NGOs) are successfully implemented
  • To support coordination between UNICEF colleagues, Government, donor partners including OMS, CDC and USAID and NGO partners, in particular for the sub-national campaigns,
  • Specific objectives - with a focus on the regions where polio cases have been identified:
  • To contribute technical assistance to the planning, implementation for field implementation, with particular attention to hard to reach populations
  • To monitor the implementation of both ongoing field activities in the regions and during the campaigns
  • To coordinate with the STOP consultant and other C4D staff doing field monitoring.
  • Provide support to NGOs to implement communication and social mobilization activities in regions where cases have been found, and support C4D coordination in the field
  • To support the implementation of community dialogue and other feedback mechanisms including a revised methodology and tools and provide capacity building and training to Government and NGO partners
  • To implement recommendations that arise from the international assessment mission in July
  • Respond to evolving needs as the emergency develops (e.g. undertaking missions if new cases are identified, revising or producing response plan documents)
  • Participate in social profile missions, as a part of case investigations
  • Support Polio outbreak response assessment recommendations implementation.
  • Contribute to update strategic documents to improve routine Immunization activities planning and implementation
  • Coordinate Regional Technical assistant interventions in the supported regions and at central level
  • Deliverables/ Results
  • At least 1 NGO for social mobilization contract effectively implemented with results for hard to reach families (remoteness and difficult infrastructure, low security areas and families in difficult social situations)
  • Field monitoring and technical support missions reports are compiled and available for internal coordination purposes
  • C4D components are available for case investigation reports
  • Community feedback dialogue methodology and tools harmonized and validated by government and civil society;
  • At least 80% of 4th and 5th Polio outbreak response assessment recommendations implemented.
  • Strategic documents to improve routine Immunization activities planning and implementation updated
  • Regional Technical assistant interventions in the supported regions coordinated and documented
  • Management and Supervision
  • The C4D Specialist will report directly to the Chief of Section, C4D

  • Qualifications, experience and competencies
  • Advanced university degree in one of the disciplines relevant to the following areas: Communication, Social science or a field relevant to international development assistance.
  • At least five years relevant C4D experience in the health sector
  • Experience with EPI programmes and communication and social mobilization for polio campaigns
  • Ability to work in a fast-changing emergency setting
  • Ability to use own initiative
  • Results-driven
  • Fluency in French and strong command of English
  • Period of consultancy
  • From 9 January to 12 Mai 2017.
